SECTION 1: OPT OUT OF COMPLETING A FREE APPLICATION FOR FEDERAL STUDENT AID (FAFSA)
A Free Application for Federal Student Aid (FAFSA) must be completed to apply for Federal and State Financial Aid. A FAFSA must be submitted every year that you want to receive Federal and State Financial Aid. Beal University offers Federal Student Aid and is available to those that qualify to include Federal Direct Subsidized Loans, Federal Direct Unsubsidized Loans, Federal Direct Graduate PLUS loans, Federal Direct PLUS loans, Federal Pell Grant, Federal Supplemental Educational Opportunity Grant, and Federal Work Study. Financial Aid is available to all that qualify.

I am notifying the Financial Aid Office at Beal University that I have filed or will file a FAFSA. I want to DECLINE the following Federal Student Aid.
SECTION 3: AUTHORIZATION TO DECLINE/RETURN/CANCEL DISBURSEMENT
This applies if you have received a 1st disbursement for Federal Student Loans during your current Academic Year. I understand that I must authorize Beal University that I want to decline, return or cancel all future disbursements. DECLINE, RETURN AND CANCEL ALL FUTURE DISBURSEMENTS.
SECTION 4: AUTHORIZATION TO RESCIND AND ACCEPT FEDERAL STUDENT AID (CHECK ALL THAT APPLY)
I authorize Beal University to rescind my prior signed authorization to decline Federal Student Aid. I want to ACCEPT the following Federal Student Aid.
Notes: Federal Direct Subsidized Student Loans: Interest is paid by the U. S. Department of Education while you are in school. No interest accumulates. Federal Direct Unsubsidized Student Loans: Interest begins accruing as soon as the loan is disbursed, including while you are enrolled in school. You must be enrolled at least half time and payments will start six months after your last date of attendance on Federal Direct Subsidized and Federal Direct Unsubsidized Loans. A Federal Pell Grant does not have to be repaid and you must meet all U. S. Department of Education eligibility requirements. There is a limit to the amount of Pell Grant a student may receive. Federal Financial Aid is available to all that qualify.
